DOH Annual Child Care Immunization Survey

The PA Department of Health requests all licensed child care providers complete the Annual Child Care Immunization Survey to meet the immunization reporting requirements outlined in 28 Pa. Code § 27.77 (a)(4) Immunization requirements for children in child care group settings. 

The Department of Health (DOH) is providing the survey below to help child care providers meet the requirements of 28 Pa. Questions should be sent to the PA Dept. of Health at RA-DHImmunize@pa.gov.  The survey must be submitted by September 19, 2025
